Dr. Louie and our associate doctors require all patients to update a health history information form at every annual eye examination appointment.  

If you prefer to fill this form out prior to your appointment, you may find and print this form under our "Patient  Forms" tab here on our website.  If you are a new patient to our practice, you are also required to read and sign our Notice of Privacy Practices, HIPPA form.  These two forms are also located on our website under Patient Forms.
DO I HAVE TO BE DILATED?
At Family Vision Care Optometrics we have always desired to provide our patients with the best quality most comprehensive care possible! In the past we have believed that a dilated retinal examination was the best method of evaluating the entire retina and ruling out the presence of retinal abnormalities and diseases such as glaucoma, macular degeneration, retinal detachments, diabetes, and high blood pressure.
We have recently incorporated the OPTOMAP Digital Retinal Imaging System into our examination procedures.  This procedure allows us to record a digital wide-angle view of the retina similar to that seen with a dilated examination. Dr. Louie and his associate doctors recommend this scan for every patient annually!
Our Staff will perform a preliminary scan during your initial testing procedures.  There is a $39 fee for adults and $24 fee for children if you and the doctor decide together to review and open the OPTOMAP scanned images of your eyes. This procedure may be covered by your medical insurance if there is a medical condition present.  If you decide not to open the images, you will not be charged.  During the examination, the doctor can determine whether or not the OPTOMAP scan can be used as a substitute for your dilated examination for that year. 
The Benefits of the OPTOMAP scan to you are:
Establishing an accurate for normal and abnormal retinal conditions with or without dilation.
Reducing the need for annual dilated examinations for certain qualified patients. It is ideal for adults and children who have difficulty with dilation.
Less inconvenience for patients who have experienced difficulties driving, returning to work or school, or completing your frame selection after their eyes were dilated.
